Health Plan is focused on ensuring timely payment processing and delivery. We want to remind you to submit an updated W-9 if your “Pay To” address has changed to ensure accurate delivery of your payment. If your “Pay To” address has changed, you will need to do the following:
- Submit a completed W-9 on the most current IRS form version (https://www.irs.gov/pub/irs-pdf/fw9.pdf).
- Send the completed W-9 to the provider services mailbox at providerservices@hpsj.com or fax it to the Provider Services Department at (209) 461-2565.
Note:
- Health Plan recommends that a W-9 be submitted whenever a “Pay To” address change occurs.
- Without exception, Health Plan can only submit payments to the address indicated on the W-9.
If you are not already a DRE provider portal user, please sign up by submitting your request through our website at https://provider.hpsj.com/public/acctdre/default.aspx
In-network providers and out-of-network providers have the option to access the Remittance Advice tool via DRE. Please review the Provider Alert posted March 22, 2024 for full instructions on how to access the RA tool via DRE.
